With the end of block 4 approaching, the Experiential Education Office would like to remind APPE preceptors about our readiness survey. The readiness survey is a required component of APPE grading which helps us gauge our students’ practice and team readiness. This data is also important for accreditation purposes.
To complete the readiness survey for your assigned student(s) at the end of each APPE block, see the instructions below:
1. Select “Intranet Login” on the Preceptor web page at https://pharmacy.wisc.edu/experiential-education/
2. Enter your Net ID and password
3. Select “Experiential Portal” to get to your Preceptor Menu
4. Select “Evaluate Student” on the menu on the left of your screen
5. Select the academic year “2026-2027”
6. Select “Evaluate/Grade” for the student you wish to evaluate
7. The two-question readiness survey will appear as a line item in the grading record. Find the “Readiness Survey (Required)” and click on ‘Survey’.
8. Answer the two questions regarding your students’ practice and team readiness using the dropdown menus (please see the screenshot below for an example).
9. Click “save” to save and submit your responses.
As a reminder, please save and close any evaluation/grading form when not actively evaluating a student. This will help adhere to FERPA rules and minimize ‘inactive’ screens from losing contact with the SoP server.
Please reach out to Sydney Taylor (setaylor7@wisc.edu) with any questions.
