If you have a block 5 APPE Student, please complete the block 5 mid-point online evaluation now and discuss with your pharmacy student. Please see the block 5 grading reminders and directions below for completing the online evaluation. Please disregard this email if you have already completed the mid-point evaluation.
Important Reminders for block 5 APPE grading:
- Final grading will close on December 23 at 12:00PM (Noon)
- Please have all block 5 grading entered by this date and time
- If APPE student is not going to mid-year, please submit all student grading information by December 16 at 11:59PM
- If APPE student is going to mid-year, all grading information will be due December 23 at 12:00PM (noon).
To complete the mid-point evaluation:
* Select “Intranet Login” on the Preceptor web page at https://pharmacy.wisc.edu/experiential-education/
* Enter your Net ID and password
* Select Clerkship-Clin Instructor to get to your Preceptor Menu
* Select “Evaluate Student Performance/Record Grades”
* Select the academic year “2024-2025”
* Select “Evaluate/Grade” for the student you wish to evaluate.
* Please note: Students are listed in alphabetical order by block.
* Scroll down and select “SPE Form”
* Evaluate the student on each performance category by clicking on the appropriate assessment per the knowledge grid at the top of the screen. Examples for each domain may be found by clicking on the “Examples” button.
* Please enter your comments in the “Comment” box especially if you are marking a low assessment. Students appreciate specific examples of their work.
* Please enter any other comments in the “Overall Mid-point Comment” box.
* To save your data, click on the “Save” button.
*To save and return to the document, click on “Save and Return”. This allows you to save your information and to return to make further edits.
* To save and release the information, click on the “Submit Final” button. The information will be released so the student may view the information and the information will be submitted to the School of Pharmacy.
* We recommend that you meet with your student pharmacist to review the evaluation.
